Chapter 12 : Electronic Health Record Systems
After reading this chapter, you should know:
- What is the definition of an Electronic Health Record(EHR) ?
- How does an EHR differ from the paper record?
- What are the function components of an EHR?
- What are the Benefits of EHR?
- What are the impediments to development and use of an EHR?
Key word:
- Meaningful Use
- Health Level 7 ( HL7 )
- Infobutton
- Health Information Exchanges ( HIE )
- Offi ce of the National Coordinator ( ONC )
- public - private keys and PKI
- HIPAA
Definitions:
- An EHR is a repository of electronically mintained infromation about an individual's health status and health care, stored such that it can serve the multiple legitimate uses and users of the record.
- An EHR, refers to the systematized collection of patient and population electronically-stored health information in a digital format.[1] These records can be shared across different health care settings. Records are shared through network-connected, enterprise-wide information systems or other information networks and exchanges. EHRs may include a range of data, including demographics, medical history, medication and allergies, immunization status, laboratory test results, radiology images, vital signs, personal statistics like age and weight, and billing information
Differ form the paper
- Inaccessibility is a problem with paper records.
- The degree to which a particular EHR achieves benefi ts depends on several factors:
- Comprehensiveness of information
- Duration of use and retention of data
- Degree of structure of data
- Ubiquity of access
Funciton components:
- Integrated view of patient data
- Clinician order entry
- Clinical decision support
- Access to knowledge resources
- Integrated communication and reporting support
Benefits:
EHR Development and use issues:
- Data Capture
- Electronic Interfaces
- Manual Data Entry
- Physician-Entered Data
- What to Do About Data Recorded on Paper Before the Installation of the EHR
- Data Validation
- Data Display
- Timeline Graphs
- Timeline Flowsheets
- Summaries and Snapshots
- Dynamic Search
- Query and Surveillance Systems
- Clinical Care
- Clinical Research
- Quality Reporting
- Retrospective Studies
